Medical Records Requests
Under the Texas Health and Safety Code, patients have the right to receive copies of their medical records. Patients must provide a written authorization to release their records that must specify the purpose of the release, the recipient, and the time period for which the authorization is valid, and in some cases proof of identity. Patient requests for medical records will be produced within 15 business days. A third-party entity handles our requests and reasonable charges may be incurred for medical record reproduction.
Medical records may be released without patient authorization in emergency situations, to public health authorities, when subpoenaed, or for billing purposes.
